Pequea Valley Athletic Department is now offering the convenience of online registration through FamilyID.  Any PVSD learner in grades 7-12 wishing to participate in school-sponsored athletics must complete the online registration form and upload a PIAA section 6 physical form from their doctor.  Registration must be completed online in order to be eligible to participate.  Please DO NOT mail any physicals to school.  If you need assistance in getting registered, please see our instruction sheet.

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.  

INFORMATION NEEDED TO REGISTER:

It will be helpful to have the following information handy to allow for accurate completion of your online registration: Primary Physician information, Insurance information · Emergency contact information, Prescription medications, and any health conditions of which we should be aware.
